Behavioral Health Case Management
At Achieve Human Services we offer Behavioral Health Case Management (Supportive Services) to our clients who may be suffering with General Mental Health/ Substance Use Disorder (GMH/SUD), a Severe Mental Illness (SMI), and/or who have experienced chronic homelessness. By providing these services we focus on independence through community living, employment, and integrative services. A Behavioral Health Technician (BHT) is assigned to each client and provides supportive services on a regular basis. The BHT will work alongside the client on a regular basis to ensure the health, safety, and self-sufficiency of the individuals being served. The BHT will complete a treatment plan with each client and by doing so they will develop specific life goals they want to achieve such as gaining employment, stable housing, addressing mental health needs, etc. The BHT will assist and encourage the client in completing the treatment plan goals. The BHT will also work closely with the client’s primary provider and attend any Adult Recovery Team (ART) meetings to ensure the client is receiving all the support needed to ensure their overall well-being.
